Employee Initiated Resignation Form

As the trust continues to improve retention of staff and improve trust internal processes an Employee Initiated  Resignation Form has been developed and launched. 

The form can be accessed within the GreenLight system and must be completed to end employment with the Trust. If you are leaving the Trust and require access to the GreenLight system to complete your Employee Initiated Resignation form please contact employment@liverpoolft.nhs.uk

The Employee Initiated Resignation Form should be completed to inform managers and HR of plans to leave the organisation. This will enable The Employment Team to correctly capture and process the required information while also allowing employees to securely and confidentially complete an exit questionnaire to help the organisation continue to improve.
