During 2024, the Trust will be replacing the current paper clinic outcome forms with an on-line version through the InTouch portal. This is part of the Trust improvement programme. The rollout schedule can be found here.
InTouch is integrated with the patient administration system and already in place for patient reception in clinics. In some areas it is also in use for patient call through to your clinic rooms.
The benefits of moving to an electronic form include
· less use of paper
· a clear and visible audit trail which provides you with assurance that your outcome requests are seen and actioned,
· better coding of MDT clinics, clinic procedures and tests to reflect the work done in clinics and their associated costs, and
· fewer omissions and transcription errors as it removes the need for manual transfer of your clinic outcomes to another system by the clinic clerks
For most clinicians, the main change will be that you will complete the E-outcome of the appointment from the clinic list in the InTouch portal, and you will not have to ask the patient to report back to reception with a paper form.
You can complete the outcomes in real time or leave it till the end of clinic, and there is the facility to look back at a clinic list to see if any E-outcome forms have been missed.
What happens next?
We will be in contact with your team 4 to 6 weeks before your clinic area or department is scheduled to move to the InTouch outcome form. We would like the change to be clinically led in your department and will be keen to work with you to help introduce the new E-outcome forms.
An e-learning module and user guide will be provided and in the initial period, members of the digital and outpatient teams will be on hand to assist with any queries.
